If you have a NDIS Plan, you may have Support Coordination funding which you will find under Capacity Building Supports.
A Support Coordinator (Level 2) will help you with the following:
Understanding your NDIS Plan and what funding you have
Help with finding the right service providers for you
Communicate with service providers on your behalf
Assist with referrals and assessments
Making sure you are working towards your goals
If you have Specialist Support Coordinator (Level 3), you will be provided with all the above and a higher level of support to help manage more tricky and complex tasks.
Throughout the engagement, your opinion and requests will be heard to make sure you are happy with the services you are receiving.

Case Management
A Case Manager will help you in a holistic way, to ensure you are connected to the right provider, not just within the NDIS service. This could be help with accessing counselling, social activities, help with housing registrations, links to health providers, etc.
